The walk to the field hospital took eleven minutes. She used three of them to count what was on the wall — carry-bag, second-station gloves, lantern hook, posted duty roster, no laminate yet, what's wrong with the laminate, what era is this glass, what era is this iodine — and used the other eight to assemble the rule that would carry her through whatever came next.

The rule was the rule she had built in the residency years when she had learned that sentiment kills more reliably than blood. Find the bleed. Stop the bleed. Do not soften the bleed by liking the patient.

The rule was also why she had been trained somewhere that did not respect a senior. She had not noticed the second half until much later.

The senior surgeon's name was Pia. Gray hair, three burns up her left forearm from a job she had not finished — a clear sign, in Renata's read, of someone who would not let her own body off the hook. Renata liked her on sight and would not let herself say so.

Pia said: What can you do.

Renata said: Triage.

Pia said: Do it.

The horns landed at 09:14 and again at 09:28 and Renata was at the receiving bay door when the first four stretchers came through.

I — The First Patient

The arterial was a mason's apprentice named, she would learn later, Ela. Twenty-four. A sword wound across the inside of her thigh. A tourniquet that had been put on by someone who knew what he was doing. The femoral, by Renata's eyes, was partially lacerated but not severed.

Pia handed her the stretcher and went for the burn. "Yours. The other two can wait three minutes."

The first stitch caught a small vessel that should not have been where it was. The vessel bled before she could clamp. The tourniquet held; nothing catastrophic; but the surgery she had budgeted at six minutes was now nine, and Pia, who had been on her way to the burn, turned around and came back.

Pia put her hand on Renata's without asking. It was a working hand — calloused, gloved, the burns above the wrist disappearing under sleeve — and the touch was not consent-floor language. It was the language of a senior who had walked into a junior's mistake before and meant to keep the patient alive.

Pia said: Show me. I will stay if you let me.

Renata's chest did the thing it had done eighteen years of medical training, twelve years of practice, three hospitals, and two countries: it tightened around the word show. She had been trained somewhere that did not show. The training had served her in rooms where showing had been weaponized.

The training had also been killing her patients at the margin.

She did not know how to say it yet.

II — The Sentence She Could Say

She said the sentence she could say.

Stop touching unless I ask. You offered to second-line — good. Consent confirmed?

It was a clinical sentence. It was also, in the only language Renata had, a thank-you. Pia, who had heard this kind of sentence from this kind of junior in this kind of room before, said nothing about it. She withdrew her hand. She held tourniquet pressure where Renata told her to. She kept gauze wet. She watched for spurting changes.

Renata reattempted the stitch.

The second attempt missed the vessel and went into tissue that did not need stitching. Bleed first, sentiment later — but the bleed was now her own pride, and the later would arrive in three minutes at the patient's expense.

She had thirty seconds before the femoral mattered.

She did the thing she had not done in eighteen years.

III — Asked

She lifted the needle off the field. She did not put it down. She held it where Pia could see it. She said, plainly:

Take it. Lead the stitch. I'll hold pressure and call vitals.

Pia took the needle.

The stitch took ninety seconds in Pia's hand. The vessel closed. The tourniquet was released at her direction. The patient's color came back at the eleven-minute mark. By 09:54 Ela was breathing well enough that Pia could pull a chair to her bedside and Renata could go to the second stretcher, where the blunt-trauma case was beginning to deteriorate in a way she had been watching for from across the room.

She worked the next four hours in the field hospital with her hands the way they had been trained, and her mouth the way it had not been. She asked Pia twice more. She asked the orderly once. She asked the apprentice nurse who could not yet read Renata's accent for what she needed and got it on the second pass.

None of the patients she touched in the next four hours died.

That was new.

That had not been new in residency, when she had been good. It had been new in fellowship, when she had stopped asking. It had been new in her first attending year, when the asking had been beaten out of her by a chief surgeon who had taught her that asking was a tell.

It was new today because she had asked.

IV — The Cloister-Yard

Pia found her in the cloister-yard at 14:10. She was sitting on the low wall with her gloves off and her hands cold from the stone. Pia handed her a cup of the wine that the cathedral cellar kept for surgeons after a hard shift and sat down beside her without asking, which Renata understood now was Pia's way of saying we have settled the consent question for today.

Pia said: You will be useful here.

Renata waited for the rest of the sentence, because the sentence was not done.

Pia said: Not at first. You have been trained somewhere that does not respect a senior. We will fix that.

Renata drank.

She did not say thank you. The sentence she had said in the operating room would have to do for now. Pia knew. They sat. The afternoon light came down through the cloister and onto the cathedral's east face and the wall they had held that morning was a quiet rectangle of stone four streets away. The mason's apprentice was sleeping in the bed Wren had paid for. The runner's mother was in the bed next to her.

Renata thought of her wedding ring, on the washstand in her quarters. She had not put it back on. She thought she would not, for a while. Not because the marriage had ended — it had, in a way she had not yet had time to grieve — but because the ring, in her glove-strip pocket, had been a thing she carried instead of asking. She had carried it for eleven years and asked nothing of it.

She would keep it on the washstand a few more days.

She finished her wine.

She went back to the hospital.
